Case Management

The Developmental Support Services Program provides case management and advocacy services to people aged eighteen years of age and older with a developmental disability and who are living independently, or moving towards independence in a community setting without significant social support.

Services are offered according to individual needs and goals and may include assistance with seeking accommodation, budget counselling, short-term emotional support, or advocacy, among others.

The Role of the DSS Caseworker:

  • Individualized planning based on the goals and support needs of the adult
  • Ongoing case management
  • Assistance to access and coordinate all necessary and available community services, including the Passport Funding Program 
 Passport Funding Program

The Passport Program helps any adult who is eligible for development services and supports funded by the Ministry of Children, Community and Social Services. It is a reimbursement-based funding program to help people be able to take part in their community's activities. Purchased participation supports can include programs, classes, camps, pre-employment, an employment supports and transportation to and from activities, among others.

To help ease the burden of expenses to our clients, where requested the DSS program can:

  • Assist with planning for the use of funding and paperwork
  • Hep with accessing supports
  • Provide brokerage services to purchase services and supports
